This Live Scan service is for UCR Personnel only.

Live Scan fingerprinting captures fingerprints electronically, without the need for the more traditional method of ink and paper. To start your background check process, follow the process below.

Complete Form & Book Appointment

Download the form and fill out the highlighted fields

  • JOB TITLE of the person coming in to be livescanned located at the top of the form
  • Department requesting the live scan
  • Contact Name – name of the person who will be receiving the clearance notification from HR
  • Contact extension
  • Section 2 of the form, containing personal data, will be filled out by the (applicant) person coming in to be live scanned.
  1. To access the Live Scan booking process, Book Now and select the "Book Now" button.
  2. Select an available Live Scan appointment.

Bring your acceptable documents and Live Scan form to your appointment.
